Registered Nurse-Caregiver Support

The Caregiver Support RN is responsible for conducting initial functional assessments, initial home visit assessments and ongoing wellness visits with Veterans and caregivers in their home environments. The CSP RN collaborates with program and collateral staff in coordinating a variety of treatment services and administrative needs including outreach, care management services, discharge planning, multidisciplinary activities, assessments, treatment planning, updating VACO-driven data for compliance and monitoring of needs. The work is primarily community-based and the selectee must be able to function autonomously and to operate a Government vehicle to visit Veterans in the community and may periodically transport Veterans in a Government vehicle. Assists Veterans and caregivers with coping and living with acute and chronic physical and mental health conditions, dealing with the loss and grief experiences of disability, terminal illness and death; documents interventions and activities in clinical record utilizing CPRS and ensures appropriate hand off when transferring of patient care. Assesses and evaluates medical and mental health needs of Veterans and their caregiving systems, forming recommendations for treatment, discharge planning, care coordination, and ensuring adherence to treatment plan. Maintains standards of patient care and documentation guidelines established by Caregiver Support Program (CSP). Establishes relationships with professional and/or other health-related groups. Provides nursing input in interdisciplinary health care plans to meet total needs of Veterans and caregivers. Maintains current knowledge of medications and therapeutic procedures. Reviews and assists in patient referrals to ensure continuity of qualified comprehensive care. Conducts assessments and care management visits with Veterans and caregivers who are participating in the CSP on a scheduled or emergency basis. Recognizes emergency situations and responds accordingly. Intercedes on behalf of Veterans and caregivers to facilitate optimum health care from VA and community providers. Participates in performance improvement activities. Provides consultation to other nursing staff, other health system services, and Caregiver support program personnel regarding Veteran and caregiver needs. Communicates goals and care needs of Veterans and caregivers to medical team and Caregiver Support Coordinators. Assists in planning and implementing CSP projects to improve and expand Veteran and caregiver services. Provides Veterans and families information and referral to community resources. Provides oversight to CSP participants and meeting requirements to ensure continued participation in accordance with the CSP guidelines. Actively participates in CSP conference calls and attends training offered by the CS; responsible for following policies and documentation as required by agency guidelines, for Joint Commission accreditation and others. Assists program leadership in making recommendations for program procedure or policy changes based upon work experiences; participates in the orientation/teaching of clinical trainees and Medical Center personnel as needed; takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested; performs accountability functions and documents all contacts with Veterans and caregivers in the medical charts as well as CARMA or other national database systems; works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing the most effective and efficient ways of delivering services to Veterans and caregivers. Position is aligned under Social Work Service and reports to the Chief, Social Work Service or designee. The incumbent will have regular access to printed and electronic files containing sensitive data which must be protected under provision of the Privacy Act of 1974 and other applicable laws, Federal Regulations, VA statues and policy. A current, valid Driver's License is required for local travel for community and home visits with Veterans and caregivers. Part time or Ad Hoc telework will be considered based on patient care and program needs. This RN position will require extensive field/community-based visitation. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. This position requires visual acuity, keen hearing, clear distinctive speech and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling and pushing. Transferring patients and objects may be required. He/she may be exposed to infected patients and contaminated materials and may be required to don personal protective equipment (PPE) in providing care to residents. They may occasionally be exposed to residents who are combative secondary to delirium, dementia or psychiatric disorders. He/she must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on Veteran need.

The check will bounce and the victim is left responsible.
